[Treatment protocol for manifestations of persistent COVID].

Persistent COVID is a symptomatic disease involving multiple organs which affects patients who have had COVID-19 and who continue to have symptoms (asthenia, arthralgia, myalgia, headache, dyspnea, cough, chest pain, dysphonia) for more than 4-12 weeks after infection. Its presentation is independent of the severity of the acute COVID-19 phase. It is predominant in middle-aged women. Symptoms frequently fluctuate. It can progress in the form of flare-ups and cannot be explained by an alternative underlying pathology. The incidence of persistent COVID is estimated to be about 10% of those infected. In addition to this symptomatic disease, it is possible that survivors of COVID-19 pneumonia may present with lung sequelae. It is probable that a high number of patients who have had COVID-19 pneumonia will require specialized follow-up by the pulmonology department after overcoming the acute phase. Different algorithms are being established to ensure proper follow-up for these patients.

Postoperative Imaging of the Wrist and Hand.

In this article we will do an overview of the general and specific complications that occur after the most common wrist and hand surgeries. Knowledge of the different surgical techniques is essential for postoperative imaging evaluation. General complications include infection, complex regional pain syndrome, problems related with the surgical approach (open or arthroscopic) and bone healing problems. The most frequent fractures of the wrist with specific complications are distal radius fractures and scaphoid fractures as associated with tendon ruptures secondary to friction, nonunion or secondary malignment. We will briefly review the different approaches for triangular fibrocartilage injuries, including acute and degenerative lesions. Scapholunate instability is the most common instability and an important indication for surgery with pin fixation in the acute setting and arthroplasty or arthrodesis in the chronic irreparable injuries. One of the most common surgeries of the wrist is carpal tunnel release, although complications are uncommon, radiologists should be familiar with the normal appearance and pathological changes after surgery. Trapeziometacarpal joint osteoarthritis is frequent especially in postmenopausal women and has several treatment options depending on the stage.

Recurrent Acute Kidney Injury Caused by Idiopathic Renal Hypouricemia: The First Report from Iran with A Novel Mutation.

Numerous factors have been involved in exercise-induced acute kidney injury (EIAKI), such as using non-steroidal anti-inflammatory drugs following exercise and idiopathic renal hypouricemia (IRHUC). IRHUC is an autosomal recessive inherited disorder characterized by impaired tubular uric acid transfer, impaired reabsorption, and accelerated uric acid secretion. Some IRHUC patients have been shown to have EIAKI. A 27-year-old police officer was admitted to the hospital due to anorexia and a serum creatinine level of 18 mg/dL, after a "tug-ofwar" game. After one dialysis sessions per day over five days, his creatinine dropped to 1.3 mg/dL. Six months later, he developed bilateral flank pain and red discoloration of urine, following a 300-meter chase of a convict, and his creatinine level increased to 2.3 mg/dL, which was corrected with proper hydration alone. Recurrent acute kidney injury can be due to hereditary renal hypouricemia, which should be considered among differential diagnoses for patients.  DOI: 10.52547/ijkd.6992.

Effect of psychological stress on the oral-gut microbiota and the potential oral-gut-brain axis.

Psychological stress in a chronic course is implicated in various diseases, such as coronary artery disease, diabetes, ulcerative colitis, and psychosomatic pain disorders. Commensal microbiota in the host tissues interact with each other and maintain overall health. Oral and gut microbiomes are considered as the most ecologically rich and taxonomically diverse microbiota communities in humans. The effects of psychological stress on the gut microbiome have been well documented, and the interaction is commonly referred as the microbiota-gut-brain axis. Like the gut microbiome, the oral microbiome contributes to maintaining both local and systemic health. Although the effects of psychological stress on the oral microbiome have been studied, comprehensive knowledge about the oral-brain axis is lacking. The oral cavity and gut can communicate with each other through the microbiota. Three-way interactions within the oral-gut-brain microbiota might exist in patients with psychological stress and disorders. The effect of psychological stress on the gut and oral microbiomes, and the potential interactions within the oral-gut-brain axis are discussed in this review.
